During registration you are required to give contact information (such as name and email address). Datazoid collects personally identifiable information from our users at several different points on our Web site. Orders If you purchase a product or service from us, we request certain personally identifiable information from you on our order form. You must provide contact information (such as name, email, and shipping address) and in some cases, you may be required to provide financial information (such as credit card number, expiration date). We use this information for billing purposes and to fill your orders. If we have trouble processing an order, we will use this information to contact you. When you place an order, you may receive confirmation email(s) or postal mail, and your order may be fulfilled online, or via Postal Mail or other courier. Application Forms If you apply for an Employer, Marketer or Reseller (collectively known as "Distributor") account on our site, we require that you provide certain personally identifiable information, including your name, company, title, address, phone number, credit card number, and in certain cases, your Social Security number. This information is necessary to process your application and to contact you. When you submit an application, you may receive confirmation(s) via email or postal mail, and your application response may be made available to you online, or via email, Postal Mail or other courier. See the “Communications from the Site” section of this statement for more information. Social Security Numbers We may require your social security number to provide certain products or services. When you enter your social security number on our registration, application or order form, we encrypt it using secure socket layer technology (SSL). Surveys or Contests From time to time, we may provide you the opportunity to participate in contests or surveys on our site. If you participate, we will request certain personally identifiable information from you. Participation in these surveys or contests is completely voluntary and you therefore have a choice whether or not to disclose this information. Uses of Information Datazoid uses the personally-identifiable information you provide to us to create and maintain card accounts, Employer accounts, Marketer accounts or Reseller accounts, to fulfill your order(s) for products or services that we provide. Non-personally identifiable information may be used in aggregate by us to track the success of marketing programs, to better understand who our customers are, and to provide you with the best possible customer service experience. We do not share non-personally identifiable information with third parties. Communications from the Site Special Offers and Updates If you so choose, we will occasionally send you information on products, services, special offers and promotions. You can sign up for these emails from us on our registration page. Out of respect for your privacy, we present the option not to receive these types of communications. Please see the “Choice and Opt-Out.” Newsletters If you wish to subscribe to our newsletter(s), we will use your name and email address to send the newsletter to you. Service-related Announcements We will send you strictly service-related announcements on rare occasions when it is necessary to do so. Customer Service Based upon the personally identifiable information you provide us, we will send you an address confirmation email, and/or a welcoming email to verify your username and password. We will also communicate with you in response to your inquiries, to provide the services you request, and to manage your account. We will communicate with you by email or telephone, in accordance with your wishes. User Profile We store information that we collect through cookies, log files, clear gifs, and/or third party sources to create a “profile” of your preferences. We do not tie your personally identifiable information, or your purchasing history, to information in the profile. We do not share your profile with other third parties, either in personally-identifiable form or aggregate form. Credit Check If you are applying for a card account, we do not request a credit report. To determine if you qualify as a potential Marketer, Reseller, or Employer program participant, we use your company name, Federal Employter ID Number, and/or your name and social security number to request a credit report. Once we determine your credit-worthiness, we destroy this document. ID/Address Verification If you are applying for a card account, we need to confirm your identity or address, using third-party services. We will use a third-party company and/or third-party software to verify your identity and address, in order to qualify you as a potential Marketer, Reseller, or Employer program participant. Enhancement of Profile Information We do not purchase marketing data from third parties and add it to our existing user database.
Information Sharing and Disclosure Aggregate Information (non-personally identifiable) We do not share aggregated demographic information about our user base with our partners and advertisers. We do not link aggregate user data with personally identifiable information. Personally identifiable information We DO NOT Share Personally Identifiable Information with Third Parties, except as noted below. Agents We use an outside shipping company to fulfill orders, and, in certain cases, a credit card processing company to bill you for goods and services. These companies do not retain, share, store or use personally identifiable information for any other purposes. Service Providers We use other third parties such as eCommLink, Inc. and our sponsor bank to provide account access and card processing services via our site. When you sign up for a card account, or register as an Employer, Marketer or Reseller, we will share as little information as necessary for the third party to provide that service. Access to your personal information is limited to only those personnel at these third parties who need access in order to maintain your account and fulfill your requests. These third parties are prohibited from using your personally identifiable information for any other purpose. Legal Disclaimer We reserve the right to disclose your personally identifiable information as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process served on our Web site. Choice/Opt-Out We provide you the opportunity to ‘opt out’ of having your personally identifiable information used for certain purposes, when we ask for this information. Co-branding partners, Marketing partners and Resellers may directly ask you for personally-identifiable information in order to initiate and maintain their business relationship with you. These partners will NOT share this information with us. Program Administrators We must share a cardholder's personally-identifiable information with a Card Program Administrator if the cardholder is part of an Employer program, Government Agency program, or other program in which card transactions must be approved by a Program Adminstrator who represents the Payor (the entity which loads the card with funds). Examples of the types of cards for which an Adminstrator has access to this information include but are not limited to Payroll Card or Corporate Expense Card (Employer has access), and Government Benefits Card (Government Agency has access). These administrators may have access to information including specific transaction data. For example, if the cardholder were to use their employer's corporate credit card, the Employer would have access to all transaction info; this is also true for programs using a Corporate Expense Stored Value Card. Program Administrators for programs in which no transaction approval is necessary — such as Merchant programs, Mall programs and the like — do not have access to the personally-identifiable information we gather. Examples of cards and programs for which the program administrator has no access to this information include but are not limited to Merchant Card and Mall Card. In general, if you are creating a card account with a company who already has access to your personal information — like your employer or government agency from which you receive benefits — the Program Administrator for that card account will have access to that same info. If you are creating a card account or accepting a card from a company that doesn't already have or need your personally-identifiable information to process your stored value card transactions — like a retailer, or a consumer goods manufacturer that is sponsoring a Promotion Card program. For example, a retailer doesn't need your personal information to deduct funds from a Gift Card. In certain cases, you may choose to provide your personally-identifiable information directly to, for example, a Retailer who is sponsoring a Gift Card. The Retailer's Gift Card Program Administrator may have access to a list of transactions involving your Gift Card, and may be able to tie that list to your personally-identifiable information you provided in order to provide you with better service and evaluate the effectiveness of a program. Before submitting your personally-identifiable information to any entity or website, you should read and understand the User Agreement / Cardholder Agreement and Privacy Policy associated with that entity, program or site. If you have a question as to whether the administrator of your card program has access to your personally-identifiable information, please review your Cardholder Agreement, or contact your Program Administrator. While we strive to ensure that those who do business work to protect their customers' best interests, Datazoid Financial Services, LLC is not responsible for the privacy practices of entities administering Card Programs that utilize Datazoid services and products. Bulletin Boards/Chat Rooms If you use a bulletin board or chat room on this site, you should be aware that any personally identifiable information you submit there can be read, collected, or used by other users of these forums, and could be used to send you unsolicited messages.
